FDA Updates Minocycline Susceptibility Breakpoints for Stenotrophomonas maltophilia Labeling Alignment
FDA updates minocycline breakpoints for Stenotrophomonas maltophilia; regulatory teams must review labeling alignment with new MIC and disk diffusion thresholds.

Labeling alignment is now the immediate action item for regulatory affairs leads managing minocycline oral and injectable product lines: FDA has published updated susceptibility test interpretive criteria for minocycline, introducing specific MIC and disk diffusion breakpoints for Stenotrophomonas maltophilia and clarifying organism-level scope for susceptibility claims.
The revised criteria establish MIC breakpoints of ≤2 mcg/mL (susceptible), 4 mcg/mL (intermediate), and ≥4 mcg/mL (resistant) for Stenotrophomonas maltophilia, paired with disk diffusion zone diameter thresholds of ≥21 mm (susceptible), 21–25 mm (intermediate), and ≤20 mm (resistant). All breakpoints are anchored to a dosage regimen of 200 mg intravenously or orally every 12 hours, a parameter that must be reflected accurately in any labeling referencing these criteria.
FDA also confirms a cross-susceptibility provision: organisms susceptible to tetracycline, specifically Bacillus anthracis, Francisella tularensis, Vibrio cholerae, and Yersinia pestis, are considered susceptible to minocycline. This provision carries direct implications for labeling language on products with claims spanning these pathogens, particularly where 21 CFR Part 201 labeling accuracy is under review.
Notably, FDA has not recognized susceptibility test interpretive criteria for Other Non-Enterobacterales or Enterococcus spp. at this time. For QA and regulatory teams, this absence is as consequential as the published thresholds: any existing labeling that implies susceptibility claims for these organism groups warrants immediate review against current FDA-recognized standards and applicable CLSI guidance.
The update reflects the ongoing evolution of antimicrobial susceptibility testing frameworks, where breakpoint revisions driven by pharmacokinetic/pharmacodynamic modeling can render previously compliant labeling outdated without a formal warning cycle. Regulatory leads should cross-reference these criteria against current approved labeling and initiate a gap assessment ahead of the next scheduled labeling review or inspection cycle.
Manufacturers with minocycline products in both oral and injectable presentations should confirm that susceptibility claims, dosing anchors, and organism scope in current labeling are fully consistent with the published breakpoint table before the next FDA labeling correspondence or post-approval supplement submission.
