Greenwich Rx Recalls Glutathione Injections Over Endotoxin Levels
Greenwich Rx recalls ten glutathione injection lots over elevated endotoxin levels, with two adverse events reported following IV administration of a subcutaneous-only product.


Two patients required emergency department care after receiving intravenous administration of a glutathione product formulated only for subcutaneous use, a sequence that now anchors Greenwich Rx's voluntary nationwide recall of ten compounded glutathione lots and raises pointed questions about endotoxin control in 503A compounding operations.
The Tomball, Texas-based pharmacy announced the recall on October 1, 2026, citing potential elevated endotoxin levels in its glutathione 200 mg/mL injection, dispensed in 10 mL multidose vials. The affected lots, spanning beyond-use dates from October 2 through November 12, 2026, were distributed to patients nationwide between June 26 and September 21, 2026. The FDA is aware of and engaged with the recall action.
The risk profile is not incidental. Injectable products with elevated endotoxin burdens carry a documented probability of causing fever, hypotension, inflammatory reactions, anaphylactic shock, and death. For QA directors overseeing sterile compounding units, the adverse event reports tied to this recall underscore a recurring gap: route-of-administration controls and endotoxin release testing must function as interdependent checkpoints, not sequential ones. A product labeled for subcutaneous use does not eliminate the endotoxin hazard if the formulation reaches an IV line.
For plant heads and compliance leads benchmarking their own programs, the lot scope here is operationally significant. Ten lots across a roughly 90-day distribution window suggests the endotoxin excursion was not isolated to a single compounding run. Bacterial endotoxin testing (BET) frequency, in-process environmental monitoring, and water-system qualification are the logical audit anchors when a pattern of this breadth surfaces in a 503A setting, where oversight frameworks differ materially from the 21 CFR Part 211 requirements governing registered manufacturers.
Greenwich Rx is notifying affected customers by mail. Patients who received product from a recalled lot are directed to discontinue use and follow disposal instructions included in the recall notification.
