Janssen Gains FDA Approval for Erdafitinib in Urothelial Carcinoma
Janssen's erdafitinib converts to full FDA approval in FGFR3-altered urothelial carcinoma, narrowing the indication and reshaping CDx and labeling obligations.

Janssen Biotech's conversion of erdafitinib (Balversa) from accelerated to full FDA approval on January 19, 2024, carries direct consequences for QA and regulatory teams managing companion diagnostic integration, amended labeling, and post-market commitments tied to a narrowed indication.
The full approval amends the prior accelerated indication, which covered susceptible FGFR3 or FGFR2 alterations following platinum-containing chemotherapy. The confirmed indication is now restricted to adult patients with locally advanced or metastatic urothelial carcinoma harboring susceptible FGFR3 genetic alterations, as determined by an FDA-approved companion diagnostic, whose disease has progressed on or after at least one prior systemic therapy, including a PD-1 or PD-L1 inhibitor. Patients eligible for, but not yet treated with, a PD-1 or PD-L1 inhibitor are explicitly excluded.
Efficacy data underpinning the conversion came from Study BLC3001 Cohort 1, a randomized, open-label trial of 266 patients. Erdafitinib demonstrated statistically significant improvements across all three outcome measures versus investigator's choice of chemotherapy: median overall survival of 12.1 months versus 7.8 months (HR 0.64; p=0.0050), median progression-free survival of 5.6 months versus 2.7 months (HR 0.58; p=0.0002), and confirmed objective response rate of 35.3% versus 8.5% (p<0.001).
For regulatory and QA leads, the CDx integration warrants close attention. The therascreen FGFR RGQ RT-PCR kit (Qiagen) served as the central laboratory assay in 75% of enrolled patients, with the remainder identified via local next-generation sequencing. That dual-assay architecture in the trial has direct implications for how sites and distributors manage diagnostic gating in commercial use, particularly where local NGS panels are deployed outside the labeled CDx. Labeling updates reflecting the narrowed FGFR3-only scope will require corresponding revisions to distribution controls and any existing promotional or prescribing materials aligned to the prior FGFR2/FGFR3 indication.
The application was granted priority review and cleared six weeks ahead of the FDA goal date, with the applicant submitting a voluntary Assessment Aid to facilitate review, a procedural detail relevant to teams benchmarking their own submission strategies under 21 CFR Part 314 expedited pathways.
Post-market obligations tied to the accelerated approval conversion, alongside ongoing adverse event reporting requirements through FDA MedWatch, represent the near-term compliance checkpoints for pharmacovigilance teams managing Balversa's commercial lifecycle.
