Lexicon Pharmaceuticals Nears NDA Resubmission for ZYNQUISTA in Type 1 Diabetes Following FDA-Defined Safety Threshold

Lexicon Pharmaceuticals' pending NDA resubmission for ZYNQUISTA (sotagliflozin) in type 1 diabetes is approaching a critical regulatory threshold: the patient exposure and safety data volume the FDA previously specified as adequate to support resubmission. For regulatory affairs leads tracking SGLT inhibitor submissions, the mechanism here is instructive, the agency's reliance on an investigator-initiated, third-party-funded study (STENO1, run by the STENO Diabetes Center in Denmark) as the qualifying safety dataset signals a continued willingness to accept non-sponsor-controlled trial data when the protocol and exposure parameters meet defined standards.
Lexicon anticipates completing NDA resubmission activities in Q4 2026, contingent on current data collection timelines from the open-label STENO1 trial. Safety data received to date from that study continue to support resubmission, the company stated. ZYNQUISTA targets glycemic control in adults with T1D, an indication for which no approved options beyond insulin currently exist in the U.S. market, a gap that has sustained regulatory and clinical interest in the SGLT1/SGLT2 dual-inhibitor class despite prior setbacks.
On the clinical operations side, Lexicon confirmed enrollment completion in SONATA-HCM, a pivotal Phase 3 placebo-controlled study of sotagliflozin in hypertrophic cardiomyopathy, with the final population substantially exceeding the 500-patient enrollment target. The study captured a majority of patients with non-obstructive HCM (nHCM), a cohort with limited existing treatment options, alongside a meaningful obstructive HCM (oHCM) cohort. Top-line data are expected in Q1 2027, with the primary endpoint assessing symptom improvement across the combined nHCM and oHCM population.
Sotagliflozin's commercial footprint is also expanding through licensee Viatris, which holds rights for all indications outside the U.S. and Europe. Viatris has secured heart failure approvals in the UAE and Bahrain and has filed in twelve additional markets including Canada, Australia, Saudi Arabia, and Singapore. Regulatory decisions in Australia and Canada are anticipated within 2026, adding supply-chain and post-approval commitment planning considerations for Lexicon's manufacturing and quality teams supporting the license.
Phase 1 development of LX9851, a first-in-class non-incretin oral compound, continues under Novo Nordisk's sponsorship, with no new data disclosed this quarter.
The Q4 2026 NDA resubmission window will serve as a concrete measure of whether the FDA's investigator-initiated study framework, as applied to SGLT inhibitor safety packages, holds under formal review.
