Par Health Recalls 22 Lots of Argatroban Injection After Multi-Material Particulate Contamination Found
Par Health recalls 22 Argatroban Injection lots over multi-material particulate contamination, including stopper fragments and possible stainless steel.

A particulate contamination profile spanning cellulose, polystyrene, polyethylene terephthalate, stopper material, and possibly stainless steel has forced Par Health USA, LLC to recall 22 lots of Argatroban Injection 250 mg/2.5 mL (100 mg/mL) to the hospital and user level, a failure signature that points directly at container-closure integrity and fill-finish process controls.
The recalled product, a direct thrombin inhibitor used in adult patients with heparin-induced thrombocytopenia (HIT) and in those undergoing percutaneous coronary intervention, carries a documented risk of life-threatening consequences if administered with particulate contamination present. The FDA's own risk classification acknowledges a reasonable probability of pulmonary emboli, vascular occlusion leading to tissue death, and thrombophlebitis. Par Health reports no adverse events to date, but the breadth of contaminant types across 22 lots distributed between December 30, 2024, and May 5, 2026, signals a systemic rather than isolated quality event.
For QA directors and fill-finish operations leads, the contaminant profile warrants close reading. Stopper material degradation points toward container-closure compatibility gaps or mechanical stress during stoppering; polystyrene and polyethylene terephthalate fragments suggest environmental or equipment-sourced ingress; stainless steel particles implicate contact surfaces in the filling line. Under 21 CFR Part 211.68 and ICH Q10 principles, each contaminant type represents a discrete root-cause pathway requiring independent CAPA documentation. A recall spanning this many lots and this range of materials will draw scrutiny to whether incoming component qualification, in-process particulate monitoring, and visual inspection protocols were operating within validated parameters.
Affected lots were distributed nationwide to wholesalers and hospitals. Par Health is coordinating returns through Inmar, Inc. (1-855-648-7909), and has issued written notification to wholesale and hospital accounts. Wholesale distributors and hospital pharmacies holding affected inventory are directed to discontinue use and halt distribution immediately; facilities that have further distributed the product must notify downstream accounts.
Adverse events and quality complaints can be reported to the FDA's MedWatch program; product-specific complaints should be directed to Par Health at 1-800-828-9393. The recall is being conducted with FDA knowledge, confirmed in the October 14, 2026 agency posting.
The multi-material contamination signature across 22 distributed lots will likely anchor FDA's inspection focus on Par Health's injectable fill-finish line qualification records and the adequacy of its container-closure integrity testing program going forward.
