Sutro Biopharma Achieves First Patient Dose in Phase 1 STRO-006 ITGB6-Targeting ADC Trial
Sutro Biopharma doses first patient in Phase 1 STRO-006 trial, its DAR8 ITGB6-targeting ADC built on site-specific cell-free conjugation.

The CMC complexity behind Sutro Biopharma's site-specific bioconjugation platform is now under clinical scrutiny, as the company doses its first patient in a Phase 1 trial of STRO-006, a DAR8 exatecan ADC targeting integrin β6 (ITGB6) in adults with advanced or metastatic solid tumors. For process development and QA teams tracking next-generation ADC manufacturing, the conjugation architecture here warrants close attention.
STRO-006 employs a site-specific β-glucuronidase cleavable linker to achieve a uniform drug-to-antibody ratio of 8, a level of conjugation precision that conventional stochastic methods cannot reliably reproduce at scale. The candidate binds ITGB6 without disrupting TGFβ signaling, a selectivity requirement that places additional demands on antibody characterization and release testing. Sutro's cell-free expression platform underpins both the conjugation control and the pharmacokinetic optimization the company cites as differentiating STRO-006 from earlier-generation constructs.
The open-label, multicenter dose-escalation study begins at 1.5 mg/kg, a starting point Sutro attributes directly to its design precision and to tolerability data from non-human primate studies at doses up to 30 mg/kg. Enrolled tumor types span ITGB6-expressing indications including non-small cell lung cancer, head and neck cancer, esophageal and gastric cancer, colorectal cancer, endometrial cancer, urothelial cancer, and breast cancer. The breadth of the enrollment population reflects the antigen's expression profile across high-prevalence solid tumors rather than a single-indication focus.
For regulatory affairs leads, the trial's design signals the CMC package Sutro will need to defend as dose escalation proceeds. Achieving consistent DAR8 across manufacturing lots, demonstrating linker stability under physiological conditions, and maintaining antibody binding selectivity are each checkpoints that will surface in any future IND amendment or BLA submission. The cell-free platform's ability to produce site-specific conjugates at clinical scale remains a manufacturing variable that inspectors and reviewers will scrutinize as the program advances.
STRO-006 is Sutro's second wholly-owned ADC to enter the clinic within twelve months, following a pipeline cadence that reflects the throughput characteristics of its cell-free conjugation system. Initial clinical data are expected in the second half of 2027, the timeframe against which process validation consistency and lot release data will need to be aligned.
The first interim safety and pharmacokinetic readout, anticipated in H2 2027, will serve as the earliest measurable indicator of whether the DAR8 conjugation strategy translates the preclinical tolerability profile into a viable clinical therapeutic index.
