AstraZeneca Wins FDA Approval for Camizestrant in Breast Cancer
FDA grants AstraZeneca accelerated approval for camizestrant in ESR1-mutation breast cancer, the first ctDNA resistance-guided oncology approval.


AstraZeneca's Etcamah (camizestrant) has cleared the FDA's accelerated approval pathway for HR-positive, HER2-negative locally advanced or metastatic breast cancer, but the approval's architecture carries obligations that will shape the sponsor's post-market manufacturing and clinical commitments for years. The agency's decision marks the first FDA approval of a cancer therapy guided by ctDNA-detected resistance mutation before imaging confirms disease progression, a precedent that extends well beyond oncology.
The indication is tightly scoped: camizestrant in combination with a CDK4/6 inhibitor (abemaciclib, palbociclib, or ribociclib) is indicated only upon detection of an ESR1 mutation during aromatase inhibitor and CDK4/6 inhibitor therapy, confirmed by an FDA-authorized test. The FDA simultaneously authorized the Guardant360 CDx assay as the companion diagnostic, meaning commercial supply and companion diagnostic availability must remain synchronized to satisfy prescribing requirements under 21 CFR Part 814.
Accelerated approval was granted on the basis of progression-free survival measured from the point of molecular resistance detection, not confirmed radiographic progression. Estimated median PFS was 16.0 months in the camizestrant-plus-CDK4/6 arm versus 9.2 months in the aromatase inhibitor-plus-CDK4/6 arm. Because the clinical meaningfulness of intervening at the ctDNA signal rather than at confirmed progression has not yet been established, the FDA has required confirmatory studies, a standard post-market commitment under the accelerated approval framework that carries withdrawal risk if those studies fail to verify clinical benefit.
For QA and regulatory leads, the boxed warning for irregular heart rhythm when camizestrant is co-administered with certain agents, alongside precautions for bradycardia and fetal harm, signals that pharmacovigilance infrastructure and labeling change management processes must be operational at launch. The Oncologic Drugs Advisory Committee reviewed the application on April 30, 2026, and the committee's deliberations on the ctDNA-guided endpoint will likely inform how FDA frames surrogate endpoint acceptability in future resistance-mutation-guided submissions.
The ESR1 mutation prevalence data embedded in the label carries supply-planning relevance: fewer than 5 percent of patients carry the mutation at initial HR-positive metastatic diagnosis, rising to nearly 40 percent after aromatase inhibitor progression, defining a commercially bounded but clinically distinct patient population that will require robust companion diagnostic throughput to identify.
AstraZeneca's confirmatory study timelines and interim readouts will serve as the near-term regulatory checkpoint against which the approval's durability is measured.
