EMA PRAC Launches Safety Review of Injectable Iron Medicines Over Hypophosphataemia and Bone Risk
EMA's PRAC has opened a formal safety referral on injectable iron medicines, citing inadequate risk controls for hypophosphataemia and bone disorders.


A formal referral procedure now under way at EMA's Pharmacovigilance Risk Assessment Committee (PRAC) puts manufacturers of injectable iron-containing medicines on notice: existing risk minimisation measures for hypophosphataemia and hypophosphataemic osteomalacia may be insufficient, and marketing authorisation changes could follow. Regulatory affairs and QA teams at affected sites should treat this as an active signal, not a background review.
The committee opened the referral after post-marketing reports documented serious hypophosphataemia cases, including osteomalacia, in patients who had received only one or two doses of ferric carboxymaltose and carried no recognised risk factors. That pattern challenges the adequacy of current product information, which limits enhanced phosphate monitoring recommendations to high-dose, long-term, or at-risk populations. Diagnostic delays compound the concern: symptom overlap between hypophosphataemia and iron deficiency, combined with the limited visibility of early osteomalacia on X-ray, has contributed to late identification of affected patients.
PRAC has broadened the scope beyond ferric carboxymaltose to encompass all injectable iron-containing medicines, citing comparable adverse event reports across the class. Oral iron formulations are excluded on pharmacokinetic grounds, systemic iron exposure from oral routes is substantially lower and the hypophosphataemia mechanism is not considered relevant at those concentrations. The procedural classification is a safety referral under Article 31 of Directive 2001/83/EC, meaning the committee will formally assess whether the benefit-risk balance remains favourable and whether label amendments or additional risk minimisation measures are warranted.
For QA directors and regulatory leads at manufacturing sites, the immediate priority is a gap assessment against current pharmacovigilance system master files (PSMFs) and risk management plans (RMPs). Signal detection protocols should be reviewed to confirm that hypophosphataemia and osteomalacia case narratives are being captured and coded with sufficient granularity to support PRAC's evidence evaluation. Sites operating under 21 CFR Part 314 with US-registered equivalents should also anticipate parallel FDA scrutiny, given the agency's history of tracking EMA class-level safety actions.
The referral outcome will determine whether changes to prescriber guidance, patient monitoring schedules, or contraindication language are required across the injectable iron class, a result that would cascade into labelling updates, artwork revisions, and potentially revised PSUR commitments for all marketing authorisation holders in scope.
