Viatris Gains FDA Approval for RYZUMVI in Presbyopia, Expanding Phentolamine Ophthalmic Indications
Viatris secures FDA approval to expand RYZUMVI into presbyopia, adding a second indication to the phentolamine ophthalmic solution via an Opus Genetics license.

A supplemental indication approval for RYZUMVI® (phentolamine ophthalmic solution 0.75%) signals how licensing-based development pathways can efficiently extend a product's regulatory footprint without a full de novo NDA cycle. Viatris received U.S. FDA approval on October 16, 2026, to add presbyopia in adults to RYZUMVI's existing indication for pharmacologically-induced mydriasis, making it the only FDA-approved product commercially available for both uses.
The approval originates from a global licensing agreement between Viatris (through an affiliated entity) and Opus Genetics, which grants Viatris exclusive U.S. commercialization rights to phentolamine ophthalmic solution 0.75%. For regulatory affairs teams, the structure is instructive: a licensor-driven development model where the rights holder carries the NDA and pursues supplemental approvals, while the originating partner retains a global licensing position. Viatris has stated it is actively exploring a U.S. commercialization partner specifically for the presbyopia indication.
Presbyopia affects a broad adult population, typically presenting in the early-to-mid 40s as a gradual loss of near-focusing ability. The condition is nearly universal with age and, when uncorrected, contributes measurably to vision-related disability and reduced productivity. The approved safety profile for the presbyopia indication identifies conjunctival hyperemia (21%) and instillation site irritation (16%) as the most common ocular adverse reactions, both reported in more than 5% of subjects in clinical trials. The majority were characterized as mild, transient, and self-resolving.
For QA and manufacturing leads, the dual-indication status introduces labeling complexity that warrants review of existing batch release documentation and any indication-specific post-market commitments tied to the original mydriasis approval. Contact lens use precautions and uveitis contraindications carry across both indications and should be reflected in updated SOPs governing product information distribution at the site level.
The commercialization partnership search Viatris has announced will be a measurable indicator of how quickly the presbyopia indication translates into market access, and whether the Opus Genetics licensing model is replicated across other ophthalmic assets in the pipeline.
