Moleculin Biotech's pivotal MIRACLE trial is approaching a critical interim readout that will shape the regulatory trajectory for Annamycin in relapsed or refractory acute myeloid leukemia, with a 90-patient interim analysis identified as the next substantive data checkpoint on the path to potential registration.

CEO Walter Klemp outlined the program's development logic in a presentation at the Virtual Investor "Why Now" on-demand conference on August 27, 2026. The MIRACLE trial (MB-108) is structured as an adaptive design, multi-center, randomized, double-blind, placebo-controlled Phase 2/3 study evaluating Annamycin in combination with cytarabine (AnnAraC). The adaptive architecture reflects direct FDA engagement following the completed Phase 1B/2 study (MB-106), which Moleculin characterizes as substantially de-risking the approval pathway.

For clinical operations and regulatory affairs leads tracking hematologic oncology programs, the trial design carries specific structural relevance. The randomized, placebo-controlled framework aligns with agency expectations for registration-enabling evidence in the R/R AML setting, where single-arm historical comparisons have drawn increasing FDA scrutiny. Annamycin's proposed differentiation rests on its anthracycline profile: engineered to circumvent multidrug resistance mechanisms and formulated without the cardiotoxicity associated with conventional anthracyclines, a pharmacological distinction that will require robust safety data packages under 21 CFR Part 312 IND reporting obligations as enrollment scales.

Moleculin also holds WP1066, a p-STAT3 inhibitor in development for brain tumors and pancreatic cancer, and WP1122, an antimetabolite targeting pathogenic viruses and select oncology indications. Neither asset was the focus of the investor presentation. The conference content was facilitated by JTC Team and Virtual Investor Co., both disclosed as paid consultants to Moleculin.

The 90-patient interim analysis will serve as the first statistically powered internal signal on whether efficacy trends observed in the earlier Phase 1B/2 cohort hold across the randomized population, a threshold that will inform both continued enrollment decisions and the timeline for any pre-NDA regulatory interactions.
